Your initial visit will take approximately one hour. When you arrive, our receptionist will ask you to complete a form for us or you can download and print the form from this page.
We will keep this form in the office. Bring along insurance information to use in completing this form. Our staff will work with you to file insurance claims and discuss financial arrangements if necessary.
Many times your insurance company will require an insurance referral to see a specialist.
This form will come from your dentist. Please check with your insurance company regarding your need for a referral, as we often cannot see you without one.

During the initial visit, we will provide a thorough oral examination, x-rays if needed and we will discuss the course of the treatment that is best suited for you. If you have any questions before your appointment, please do not hesitate to call our office.

How do I know if I have gum disease?

If you notice any of these warning signs, come and see Dr. Porras:

• Gums that bleed during tooth brushing
• Red, swollen, bleeding or tender gums
• Persistent bad breath
• Pus between the teeth and gums
• Loose or separating teeth
• A change in the way your teeth fit together when you bite
• A change in the fit of partial dentures
• Your teeth were intended to last a lifetime—and they can, if cared for properly
